Forms info:
Health Forms- can be brought in to the health staff on schedule pick-up day (8/8), or dropped in the drop box on our building, to the left of Door 1. The box is checked several times a week.
New Registers or if you have moved but still live in our boundaries- Proof of residency forms and birth certificates MUST be brought in to our registrar, Lindsay Nauss, to have copies made. Please call to make an appointment after July 29th, or email Lindsay at lindsay.nauss@cusd200.org after July 29th.
