Physical and Dental Exams

Students entering kindergarten, fifth grade and ninth grade are required to submit proof of physical examination and required immunizations.  All students new to District 200 must also meet this requirement. All physicals must be completed and on file before a student may participate in physical education classes.  Parents should be reminded that students will be excluded from school attendance if their physicals are not on file by September 15.  State law also requires students in kindergarten, second grade and sixth grade to have a dental exam by May 15, 2007.  Click here to download a copy of the physical form or click here to download a copy of the dental form.

Free and Reduced Price Lunch and Medical Insurance

Students may apply for free or reduced price meal services by completing the application provided in each school’s office.  Click here for more information about the free and reduced lunch program.  Children eligible for free or reduced price meal benefits may also obtain free or low-cost health insurance through the Illinois Children’s Health Insurance Program, KidCare.  Information on this program is also available in the school’s office.

Hot Lunch Program Offered

Hot lunches are offered in all elementary schools by Sodexho, the District’s food service provider.  New this year is a choice of multiple hot and cold entrees and a fresh garden bar.  A computerized point of sale system is used to track each student’s account, including the balance and the meals the child has chosen.  Regular meals, including milk, will cost $2.20.  Milk alone is $.30. More information is available in each elementary school.

BASP Programs

All elementary schools offer before and after school programs for interested families. The programs operate from 7-9 a.m. and from 3:30-6 p.m.  There is no program for kindergarten students during regular school hours. The cost of the program is $8.50 per session or $17 a day.  Information about each school’s program may be obtained from the school office.
